MORTGAGE APPLICATION CHECKLIST

Application Checklist

In order to expedite your loan application, it is important that you supply all of the applicable items listed on this form. Please bring copies for your Loan Officer when you come in for your loan application. Below is the list for most FULL DOC loan programs:

Check Original Sales Contract: Signed by Buyers and Sellers

Check Check for Application Fee: Applied Towards Appraisal and Credit Report

Check Residence for the Past Two Years: Provide Account Number, Name and Address of Mortgage Holder - If Renting, Provide Name and Telephone Number of Landlord.

Check Employment for the Past Two Years: Bring W-2's for the previous two years and provide most recent month's worth of paycheck stubs along with the name and address of employer. If self-employed or commissioned, provide past two years' tax returns with all supporting schedules and a year-to-date financial statement or profit and loss statement.

Check Two years of tax returns all pages, all schedules. Have the third year on standby. They need to be signed and filed - the lender will check them against IRA records (4506T)

Check Outstanding Loan and Liabilities: Provide Creditor's names, addresses, account numbers, minimum monthly payment, and estimated current balances.

Check Assets: Checking, Savings, Securities, 401K, IRA, etc.: Bring last three monthly statement, account numbers, name and address of institution and current balances.

Check Copy of Divorce Decree: Provide this if you are divorced to show alimony or child support received or paid out. Must provided divorce petition, final decree and any subsequent amendments to show alimony or child support received or paid out.

Check If you have deferred student loans copies of the deferment letters for all accounts showing at least 12 months deferment.

Check If you are receiving social security or disability income we need the award letter showing you will be receiving it for at least 3 years into the future.

Check Name and contact information for your insurance agent or company.

Check Other Items: To use alimony or child support as income, please provide 12 months canceled checks or bank statements showing deposits. To use rental income, provide a current one year executed lease. If relocating, provide a copy of relocation agreement and company contract. If you are a VA qualifying Veteran, provide Certificate of Eligibility and DD-214.

Check Copy of Driver's License and Social Security Card: In applying for an FHA or VA loan, provide a copy of the driver's license and Social Security card on each applicant. If you need a replacement license, you may obtain one by contacting the Department of Motor Vehicles in the issuing state and provide them with any two of the following: birth certificate, passport, social security card, voter registration card, last year's Federal Income Tax Return or marriage license. If you need a replacement social security card, contact any Social Security office to get a verification of receipt of you current account number. You may also contact the Security Administration at 1-800-722-1213 to obtain the same information.
